Coverage of IVF, GIFT and ZIFT is required only if: the treatments * The patient has used all reasonable steps, less expensive and medically appropriate and still could not get pregnant or carry a pregnancy; * The patient has not reached the maximum number of queries egg * The procedures are performed in facilities that meet the standards set by the American Medical Society Reproductive, or the American College of Obstetricians and Gynecologists.
