What is the criterion for policy health insurance state farm life perferred rates and super favorite?

I am considering purchase a life insurance policy through State Farm which have different rates depending on the results of a physical examination to be performed. The agent can not say, but I want to know is what are the criteria for qualifying for preferred rates and super preferred. BMI? (BMI), blood pressure? cholesterol level? My goal now is to find out if I qualify and if not, but I'm around I could work on improving my health over the coming months diet and exercise. You have to be in good health. What I suggest is to do this: Tell your agent to submit your request "does not Bind".
